U.S. which keeps workers' compensation records. Plus, you get
detailed instructions on how to retrieve the information you
need.

Records are availabe
for the past 8 years by mail and in person. All requests must
be made in writing. Claimant's attorney must have contract or
medical authorization. Employer/carrier must be party to action
to obtain records. They do not conduct |
searches for pre-employment
screening. In person, anyone can come in to view. Medical information
is not made available and copies cannot be made unless there
is written authorization. Search information required: full name,
SSN, docket number, EIC. $.50 per page fee for copies. $1.00
certification fee. Fees payable by business or personal check
only to Mississippi Workers' Compensation Commission. They will
invoice. The turnaround time for mail searches is 3 to 4 working
days. |
DRIVING RECORDS
The Guide lists the addresses and phone numbers
of every motor vehicle department in the U.S.. Plus, instructions
and procedures for obtaining driving records which allow you
to verify legal name, date of birth and other essential data.

Records are availabe
for either a thirty-nine month record or for a five year record.
CDL records may be available for ten years. Access is by mail,
in person and via computer. Overnight magnetic tape-to-tape ordering
is available. Driver's address is included. In person, there
is a limit of 2 requests per trip to the window. Computerized
access: Arizona offers an interactive on-line system. For |

required: full name, DOB, driver's license number. Certain exempt
requestors (identified by law) need only supply 2 out of the
3 items required to search. Any person requesting a motor vehicle
record shall identify himself and state the reason for the request.
ID may be required. The current fees are $3.00 for 39 month records
and $5.00 for certified 5 year records. Insurers may only receive
the 39 month record. Fees payable by business or personal check
and in state check to Motor Vehicle Division, Record Services.
Prepayment is required. The turnaround time for mail searches
is 1 week to 10 days. |
FEDERAL COURT RECORDS
The Guide's Federal Courts Directory contains
addresses and phone numbers for each of the nation's 93 Federal
Districts. Now you can obtain federal criminal convictions data,
civil litigation records and bankruptcy filings and dispositions.
Plus, you receive information on how the Federal Court System
works and how to access its records for your investigation.
